Dr. Fields is the project investigator for South Carolina’s Youth Access to Psychiatry Program (YAP-P) and provides consultation to primary care providers seeking mental and behavioral health guidance for their young adult patients. She is a board-certified psychiatrist and the Director of Integrated Care for the SC Department of Mental Health (SCDMH). She has more than 20 years of clinical experience and expertise in integrated care and community mental health. Dr. Fields has led organizational efforts to implement a variety of integrated care clinical and training programs. She graduated from the University of Maryland School of Medicine, where she completed an internship in internal medicine, and she completed her psychiatry residency at Johns Hopkins Hospital. She is a clinical associate professor of Neuropsychiatry and Behavioral Science at the University of South Carolina School of Medicine, Columbia.
Grace is the Project Director for YAP-P and has over fifteen years of experience in health and human services. She began working with SC Department of Mental Health (DMH) as the Integrated Care Manager but transitioned her role to Project Director of YAP-P in 2022. Prior to working at DMH, she was the Director of the Medical Management Oversight Division at the SC Department of Public Health, the Director of Planning, Research and Evaluation for the Virginia Board for People with Disabilities, the Senior Consultant at the South Carolina Department of Alcohol and Drug Abuse, and the State Director for the South Carolina Nurse Family Partnership program. She holds a master’s degree in healthcare administration from the Medical University of South Carolina, a master’s in public administration from the University of South Carolina and is a certified Project Management Professional (PMP). Grace is also the mother of three grown sons.
Sarah is the Marketing and Outreach Coordinator for YAP-P. She is a Licensed Master Social Worker (LMSW), graduating with her MSW from the University of South Carolina, and B.A. in Psychology from the College of Charleston. She has previously worked in the veteran space at both the state and non-profit level and has experience in food related needs and school behavioral health research. Now, with YAP-P, she coordinates the monthly newsletter, digital marketing initiatives, and outreach efforts. When she’s not working, she enjoys all things artistic-especially painting, reading, and hiking.
Ezra Moore is a program/operations coordinator, responsible for ensuring the consultation line is working correctly: from calls being answered and consultations being scheduled, to technical support, quality improvement, writing operational procedures, and procurement. Ezra has a Bachelor of Science from Strayer University for Information Systems with a Project Management Concentration. Originally from Clifton Virginia, he moved to Greenville SC in 2014. His interests include writing novels, teaching, listening to music, playing video games with his loved ones, and being a dedicated cat dad.
Sharyl is the Intake Coordinator with YAP-P; she is always eager to facilitate setting up consultations for you with our specialists in child psychiatry and developmental behavioral pediatrics. She has over two decades of entrepreneurial experience in the private sector, interspersed with experience working as the Assistant to the Abbeville County Chief of Police, an Office Administrator for Hall Utility Company, and Event and Hospitality Manager for Camp Fellowship on Lake Greenwood, SC. She holds an associate’s degree in business from AB&F Technical College. Outside of work, Sharyl is passionate about volunteering with hospice facilities, children’s camps, and enjoys spending time with her husband outdoors.
